The reward will be given if ALL of the following requirements are met:*

1. Information is provided directly to the Riverside County District Attorney’s Office which is unique, specific,
    previously unknown, and not currently under investigation.

2. If more than one claimant is eligible for this reward, the individual offering this reward may apportion the
    reward money in a manner that individual deems appropriate.

3. The capture and arrest of Elva Diaz for the charges below are a direct result of the information provided:

       2.  PC 191.5(A)  F  DUI Gross Vehicular Manslaughter 02/21/2008
       3.  VC 14601.2(A)  M  Driv Susp/Revoke CDL DUI/Drugs 02/21/2008
